MI Choice Medicaid Waiver Program

The MI Choice Medicaid Waiver Program is a home- and community-based program that provides in-home services for eligible older adults and adults living with disabilities. Long-term support and services are designed to allow recipients to stay in a home or residential setting (including an individual’s or family member’s home, assisted living communities, licensed homes for the aged, or licensed adult foster care homes). The program is funded through the Michigan Department of Health and Human Services.

Our staff members work with you and your family to give you more control over the care you receive. Services include adult day care, community living support, respite care, home modifications, equipment, meals, transportation, and personal services. The program is provided at no cost to eligible adults.

There are financial and medical eligibility requirements for the program.

Care Management / Case Coordination and Supports

The Senior Alliance provides Care Management and Case Coordination and Supports for eligible adults and adults with disabilities. These home-and community-based programs assist adults in remaining in a home setting such as their own home, a family member’s home, or an unlicensed assisted living community.

The programs are funded through the Michigan Aging and Adult Services Agency.

Modeled after the success of our community-based Care Transitions program, Homecomings provides a patient-centered approach to working with individuals and their families, from their hospital admission through their 30-day post-discharge period and back into their home.

We understand that the patient should be at the heart of patient care. That’s why we work to help empower patients to take a proactive approach to manage their health needs, with the goal of avoiding readmission to the hospital for the same or related reasons. We strive to provide this enhanced level of service with the utmost quality of care and benefit. By helping patients avoid readmission within 30 days of discharge, Homecomings not only eliminates avoidable medical expenses for the hospital and the patient, but it also assists with identifying other social determinants in the patient’s home that can be corrected or improved to enhance the patient’s overall quality of life.
